What is OpnForm?

OpnForm is an open-source form builder for surveys, registrations, applications, intake processes, and internal workflows. It supports unlimited forms and submissions, logic and validation, calculations, prefilled fields, URL parameters, file uploads, webhooks, API access, and reusable templates.

Forms can be hosted by OpnForm or self-hosted, shared through a link, or embedded on a website. Higher tiers add branding control, domains, messaging integrations, access protection, analytics, AI generation, and workspace options; administrators must still configure retention, consent, access, and data-processing practices for the information collected.
